Case Study #1: Stephanie F.
Chief Concern: 65 year old female with a 20 year old upper crown and bridge fixed restoration that was “falling apart,” due to dental caries and periodontal disease. She wants to replace her teeth with a fixed dental implant bridge.
Treatment Received: In one surgical visit, the teeth were removed and implant site development was completed with bilateral lateral wall sinus augmentation (vertical bone reconstruction) followed by 8 implants 12 months later. She wore a transitional removable denture during the healing phase. The final case is a fixed bridge attached to 8 implants with a night guard appliance made to protect her “investment.”
Case Study #2: Arlene F.
Chief Concern: 69 year old presents with concerns of her needing to lose her lower right first molar (#30) due to an abscess. Her lower left implant #18 was previously placed by Dr. Levine a few years prior.
Treatment Received: In surgical visit #1, the tooth was removed by Dr. Levine and ridge preservation was completed (bone graft, membrane, and PRGF: plasma rich growth factors – from blood drawn) to preserve the ridge in 3 dimensions. Three months later a site-specific CBCT was taken in-office to finalize implant position and length in relation to the mandibular nerve. A surgical guide was fabricated, the implant was placed, and the case was completed by her restorative dentist 8 weeks later.
Case Study #3: Judy N.
Chief Concern: 55 year old female presents with broken tooth #4, which is non-restorable on a previously crowned and root canaled tooth.
Treatment Received: In one surgical visit, extraction of #4, site specific sinus bone augmentation (osteotome procedure) with single dental implant placement. The porcelain crown was completed 5 months later for #4.
Case Study #4: Nancy M.
Chief Concern: 61-year old who presents with multiple sets of ill-fitting dentures. The upper jaw had receded significantly requiring extensive bone reconstruction surgery.
Treatment Received: In two surgical visits, implant site development was completed in her upper jaw using human adult stem cell allograft (Osteocel®) and titanium mesh as a scaffold. Five implants were then placed in 6 months followed by completion of upper case in 4 months with night guard appliance.
Case Study #5: Mary K. 56 Year Old
Chief Concern: Patient has been unhappy with her esthetics and comfort of a previous full mouth reconstruction which was supported on failing implants in her upper right, upper left and lower left sections.
Treatment Received: After removal of failed implants and due to bone atrophy and bone loss in 3 dimensions, vertical and horizontal ridge reconstructions were completed using titanium mesh, bone grafting and plasma rich growth factors in her upper right, upper front and upper left areas along with bilateral sinus augmentation procedures (lateral wall approach). 6 implants were placed in her upper arch and 5 in her lower left arch which was treated as a SameDay Smile®. Pink porcelain was used in her final restoration due to significant vertical bone loss. A night guard was fabricated to protect her porcelain restorations.
Case Study #6: Pat B.
Chief Concern: 69 year old female aware that she will be losing use of her upper left 4-unit fixed bridge due to advanced periodontitis.
Treatment Received: After periodontal nonsurgical therapy and in one surgical visit, all 3 abutment teeth were removed (#12, 13, 15) and immediate dental implants were placed in sites 12, 13, 14 with site specific sinus bone augmentation procedures (osteotomes). The new fixed bridge, 12 thru 14, was completed 6 months later.
Case Study #7: Tatyana B
Chief Concern: 63 year old female presents with being very unhappy with her smile, very loose teeth and inability to function with a removable partial denture.
Treatment Received: Periodontal therapy was completed in the lower arch and in one surgical visit, removal of her remaining upper teeth and bilateral lateral wall sinus augmentation. A transitional complete upper denture was worn during the healing phase followed by the placement of 6 dental implants. The case was completed as a porcelain fused to metal 1 piece prosthesis with pink gingiva.
Case Study #8: Max K
Chief Concern: 42 year old male who was very unhappy with his smile, loose teeth and generalized advanced periodontitis.
Treatment Received: In one surgical visit, all remaining teeth were removed with placement of 6 implants in his lower arch for a SameDay Smile®. Ridge preservation was completed in the upper jaw with bone grafting to preserve remaining bone contours. Bilateral lateral wall sinus augmentations were completed in the upper jaw 8 months later followed by 8 dental implants. His case was completed with a framework reinforced doctor removable hybrid restoration in both arches.
Case Study #9: Jordan S. 53 Year Old
Chief Concern: Patient was unhappy with his smile, worn teeth and failing dentistry and desires a “Smile Makeover.”
Treatment Received: After controlling his periodontal disease, orthodontic therapy was completed to align his lower teeth, sinus bone grafting (lateral wall approach) was performed in his upper right posterior for implant site development, 4 teeth removed and 4 dental implants placed (2 upper right, 2 lower left). Final single crowns were placed for all upper teeth.
Case Study #10: Audrey F.
Chief Concern: 59-year-old female presented with two, failing, endodontically treated, maxillary central incisor teeth (#8, 9). She was very unhappy with her smile and was unable to bite on these teeth due to discomfort and tooth mobility.
Treatment Received: Both teeth were removed and implant site development was completed with hard and soft tissue augmentation. Single implants were placed three months later followed eight weeks later by fixed provisional crowns for tissue sculpting, followed by her final single crowns. Orthodontic therapy in lower jaw is nearing completion to allow the placement of single implants in her first molar areas.
